Benprost, bosentan, etc

Bosentan (bosmon tablet) for pphn
iNO is first line
Then milrinone
Then may add sildenafil with milrinone
Then comes bosentan (bosmon)
And benprost (beraprost) PGE2 agonist

And 
for keeping pda open, 
its PGE1 alprostadil 0.01 ug per kg per minute 
Then comes
Benprost (beraprost)



Milrinone
Term infant Preterm infant
Maintenance without loading dose
0.33 − 0.75 microgram/kg/minute 0.2 microgram/kg/minute

Or loading with 75ug/kg/minute over 1 hour

Susp Silagro 10mg/mL
1mg per kg per day divided 8 hrly
Sildenafil
Pulmonary hypertension Initially 0.25 to 0.5 mg/kg/dose every 4 to 8 hours, adjusting according to response.

Bosentan (tab bosmon 62.5 mg)
1 mg per kg per day divided 12 hourly
0.5-2 mg/kg/dose 12 hourly.
Suggested regimen: Commence at 0.5 mg/kg/dose 12 hourly. Increase incrementally in consultation with
neonatologist and/or cardiologist to a maximum of 2mg/kg/dose 12 hourly.

1ug/kg/dose 6 hrly
TO KEEP DUCTUS ARTERIOSUS OPEN
It is used for pulmonary hypertension also.
